GHFA Seeking Applicants For Second Cycle 2022 Community Health Grant Awards Program

The Georgia Healthy Family Alliance (GHFA) is currently seeking applications for the second cycle of the 2022 Community Health Grant Awards program. Grant awards of up to $5,000 are made to GAFP member-sponsored community groups that support program priorities including underserved populations and community projects that promote healthy practices consistent with the principles of Family Medicine. Current GAFP members including medical students, residents and active/ life members are eligible to apply.

The application deadline for second cycle 2022 awards is May 14, 2022. Second cycle grant awards will be announced in June 2022.

Over the last ten years, the Community Health Grant Program has given over $500,000 in grants back to GAFP members, residency programs, FMIG groups and their communities including Wellstar Kennestone Family Medicine for the “Let’s Get Moving” Pedometer Project:                                                        

 ” I am motivated to live healthier for my children. I want to control my diabetes through exercise.” A Wellstar “Let’s Get Moving” Pedometer project participant on what motivates her to make healthy nutrition and exercise choices.

GHFA Awards $10,000 in First Cycle 2022 Community Health Grants To Support Local Health Projects

The Georgia Healthy Family Alliance (GHFA) awarded Community Health Grant Award applicants $10,000 in first cycle 2022 grants.  Grant awards were made to GAFP member sponsored community groups that support GHFA program priorities including underserved populations and programs that promote healthy practices. The application deadline for second cycle grants is May 14, 2022.

Clarkston Health Center, “Breast Health Initiative” Patrice Shongo, MD $5,000

Clarkston Health Center’s (CCHC) Breast Health Initiative (BHI) aims to decrease the breast cancer mortality rate among uninsured, immigrant, and refugee women in DeKalb County and the surrounding region. In partnership with Emory-Decatur Hospital, CCHC has been providing free mammograms and follow-up, diagnostic testing for women aged 40 years and older who are patients of the clinic; or eligible women who are referred from elsewhere. During 2019-2020, 188 women received screening mammograms. Of the 188 women, 75 had benign results, 44 were submitted for follow-up, 11 biopsy orders were sent, 8 ultrasound-guided biopsies were completed, 9 genetic orders sent, 16 unilateral diagnostic mammograms completed, and one bilateral diagnostic mammogram was completed. CCHC’s volunteer physicians and nurse practitioners continue to conduct the initial clinical breast exams, identify any risk factors through a detailed medical history, and order the testing. Our funding will provide mammograms for an additional 210 women in 2022.

Physicians’ Care Clinic “Chronic Disease and Diabetes Management Program” Martha Crenshaw, MD Dekalb County, $5,000

Physicians’ Care Clinic (PCC) provides low income, uninsured adults who reside in DeKalb County with quality, comprehensive, nonemergency medical care. PCC currently has 1,413 enrolled patients, 80 of whom were new to PCC in 2020-21.  Services provided by PCC include chronic disease management, disease prevention and health education, access to primary care physicians, referrals to medical specialists, pharmaceuticals, and health management supplies. The clinic operates with a small paid staff and 217 health care professionals who volunteer including physicians, nurses, pharmacists, laboratory technicians, and others who provide in-clinic medical services. This project focuses on diabetes care and management, as well as heart disease and stroke prevention. PCC provides general cancer screenings, such as breast, colon, prostate, cervical, and endometrial. In addition to these screenings, the project provides laboratory screenings for high blood pressure, elevated A1C, elevated lipid panels, preventative eye examinations, podiatry exams, education classes and monitoring supplies.

Southeast Georgia Strike Force Tar Wars Launched in Savannah

Statewide Manager, Tar Wars Education Abbey Carr and PE and Health Program Coordinator Ashely Tremble-Scott organize kits for school distribution at the SCCPSS Central Office.

The Georgia Healthy Family Alliance is raising an initial fundraising goal of $250,000 in the Savannah/Statesboro (Southeast) Georgia Strike Force.  While we are over half-way to our fundraising goal, we have already launched our efforts to divert children from ever taking their first puff of any tobacco or vaping products.

At the beginning of March, 123 Tar Wars classroom kits were hand-delivered to the Savannah Chatham County Public School System Board of Education offices to be distributed to 30 schools across the district.  By the end of March, 3,690 4th and 5th grade students will participate in our tobacco, nicotine and vape prevention program given by over 30 Tar Wars trained physical education and health educators throughout the school system.

Georgia Healthy Family Alliance Welcomes Alexis Klima as Director of Philanthropy

The Georgia Healthy Family Alliance welcomes Alexis Klima as director of philanthropy effective March 2022. Alexis is a philanthropy and marketing professional with more than 13 years of experience managing and growing programs, as well as increasing exposure, involvement and revenue for organizations. She is skilled at recognizing and motivating potential in others and translating philanthropy, marketing and business ideas into strategic action.

As director of philanthropy, Alexis will support the Your Giving is Great Medicine capital campaign aimed at expanding the reach and funding of the Tar Wars Tobacco and Vaping prevention program statewide, as well as increasing support for Community Health Grants and Immediate Needs Grants in communities where GAFP members live and serve.

Alexis’ most recent philanthropy position was as the development director for the Children’s Home Society of Florida within the Sun Coast (Tampa, St. Pete and Clearwater) and Lakeland regions. Within this role, she conducted the full spectrum of donor development – cultivation, solicitation, stewardship and donor recognition. She led fundraising efforts for annual and major gift donors, donor-centered events, committee management, corporate sponsorships, foundation support, media relations and local board development.

Alexis earned a MBA with a specialization in Marketing from Florida Institute of Technology and a Bachelor of Arts in Mass Communication from Georgia College & State University.

Georgia Healthy Family Alliance Welcomes New Trustees

The Georgia Healthy Family Alliance (GHFA) welcomed two new members to the Alliance Board of Trustees in 2022. Christina Kelly, MD, FAAFP of Fort Benning joined the GHFA Board for a four-year term and Dr. Alexandria Poitier of Atlanta was elected to serve a two-year term as a Resident board member.

Originally from Akron, OH, Dr. Christina Kelly was awarded her medical degree by the Ohio State University College of Medicine and Public Health. As an Army wife, Dr. Kelly has worked in various other settings – as an employed physician practicing full-scope family medicine including obstetrics, and as faculty at family medicine residency programs in Washington, Texas, Georgia and North Carolina. Currently, Dr. Kelly serves as Assistant Professor in the Department of Family Medicine of the Uniformed Services University of the Health Sciences.

Having presented Tar Wars for the first time in 2003, Dr. Kelly has been active with the AAFP and the AAFP Foundation throughout her career and looks forward to continuing her advocacy work while serving on the GHFA Board. “Watching the videos where patients told their stories about how they were helped through the Community Health Grant Program showed me what a great service GHFA is to the state and how important this work is,” Dr. Kelly said.

New Resident board member and Florida native, Dr. Alexandria Poitier earned her medical degree from the University of Miami. She is currently a PGY1 Resident at Morehouse Family Medicine Residency Program where she serves on the Program Evaluation Committee.

As a GHFA Resident board member, Dr. Poitier looks forward to sharing information about  Alliance programs with her peers. “Participating on the GHFA Board allows me another opportunity to engage in projects to improve health literacy and decrease barriers to accessing care for vulnerable populations,” Dr. Poitier said.

GHFA board members help guide and oversee Alliance projects including the Community Health and Immediate Needs Grant Programs and the Tar Wars vaping and smoking prevention program.  GHFA also currently has three Tar Wars Strike Force Initiatives underway in Rome, Gainesville and Savannah to address the epidemic of youth vaping in those communities. Plans are currently underway to launch a middle Georgia Strike Force in Macon this year.

Looking for a way to be more involved with your academy?  Want to give back but aren’t sure where to start? The Georgia Healthy Family Alliance (GHFA) is seeking an interested GAFP member to serve on the Alliance Board of Trustees beginning in 2021.

Formerly the Foundation of the GAFP, the Alliance was founded in 1987 and is the philanthropic arm of the Georgia Academy of Family Physicians. The mission of the Alliance is to enhance the well-being of Georgia citizens through educational and community programs that promote healthy practices consistent with the principles of family medicine.

 

Now, more than ever the Alliance needs family physicians who are excited about our future focusing on our Capital Campaign that is nearly halfway to raising $4 million dollars.  The campaign will allow the Alliance to expand its educational and community outreach efforts including the Community Health Grant Program, Immediate Needs Grants for COVID-19 assistance and the Tar Wars tobacco and vaping education program.  The campaign will also provide the Alliance with a full-time team dedicated to improving the health and welfare of our local communities in Georgia. There is an expectation that new Trustees will make a contribution to GHFA’s Capital Campaign in an amount that is meaningful to them.

New board members will serve four-year terms (2021-2025) and will help guide and oversee GHFA projects including:

  1. The Community Health Grant Program – This award winning (2015 AAFP Foundation “Program of the Year”) program awards grants to GAFP member affiliated projects addressing health care needs throughout Georgia. Since beginning this program in 2012, the Alliance has awarded over $365,000 to community projects in all corners of the state. In 2020, the Alliance also launched the Immediate Needs Grant program to provide urgent needs assistance in response to the COVID-19 crisis. Since launching in March, $35,000 has been awarded statewide and the program was named the 2020 AAFP Foundation Outstanding Program of the Year.

 

  1. Tar Wars – a tobacco and vaping education program presented to more than 3,000 fourth and fifth graders in Georgia during last school year alone. This year the Alliance will also launch a Southeast Georgia Tar Wars Strike Force anti-vaping initiative in addition to the two already underway in Rome and Gainesville to address the epidemic of youth vaping in Georgia.

The Georgia Healthy Family Alliance Awards $25,000 in Third Cycle 2020 Community Health Grants to Support Local Health Projects

The Georgia Healthy Family Alliance (GHFA) awarded four Community Health Grant Award applicants $20,000 in the third cycle of 2020 grants. Grant awards were made to GAFP member affiliated community organizations that support GHFA program priorities including underserved populations and programs that promote healthy practices. Third Cycle 2020 Community Health Grant Recipients are:

Adult Disability Medical Home “Get Fit & Be Healthy Cookbook and Health Guide” – $5,000 Sponsored by Andrea Videlefsky, MD

The Get Fit and Be Healthy program has been a hallmark health and wellness program offered by ADMH to individuals with developmental disabilities. Over the past three years the ADMH has expanded their patient population to include multiple complex developmental disabilities including Autism, Fragile X, Microcephaly, and Cerebral Palsy. Through interactive group visits with peers along with caregiver support, this program will provide continued and expanded health services to include both ADMH patients and other eligible individuals in the community… The program provides an opportunity for individuals with developmental disabilities to actively engage in a nutrition and fitness education program to develop lifelong habits for improving and maintaining health and wellness. Using the tools and strategies from previous cohorts to develop a creative and long-lasting outcome, a cookbook/health manual incorporating health and fitness information with recipes learned in the program will be produced. The manual will be participant driven, developed in workgroups based on the three GET FIT program components Health Tools, Fitness Training and Healthy Recipes.

“Warrior Centric Healthcare Foundation & Veteran Medical Neighborhood Joining Forces to Mitigate Impact of Social Determinants of Health” – $5,000 – Sponsored by Loretta Hicks, MD

There are 18.8 million veterans living in the U.S. today, 7.6 percent of the population. They are predominantly male although the number of female veterans is on the rise. Georgia is home to over 636,725 Veterans, 69,836 active duty members and 27,557 National Guards and Reservists and together their family members total over 1.9 million. There are just under 75, 000 females and 563,253 male Veterans. In addition, figures 1 thru 3 provide a graphic of the age, ethnicity, and era of service.

The goal and objective of the Warrior Centric Health Foundation – Veteran Medical Neighborhood Social Determinants of Health (SDoH) technology platform project is to provide an efficient, effective, and economic mechanism for family physicians to address the SDoH facing Veterans and their families who are in their care. This tool will be available to all members (physicians and other disciplines) of the GAFP. It will also be available to any patients they direct to the site. Once the integration is complete, they will be able to access the tool through the WCHF website at no cost to them or their patients.

Mercy Health “Flu Prevention & Treatment for the Underserved Project” – $5,000 – Sponsored by Mitch Cook, DO

Mercy Health Center provides quality, whole-person healthcare at no cost to people who are low-income and uninsured. Patients served are 18-64 years old and live at or below 150% of the federal poverty guideline. Mercy serves 6 counties which include Barrow, Clarke, Oconee, Oglethorpe, Madison, and Jackson. The patient demographics are as follows: 52% Caucasian, 23% African American, 22% Hispanic, 3% other and 63% of patients are female and average age of all patients is 45 years old. In 2019, Mercy served 1,448 unique patients for a total of 11,447 visits. Grant funds will help provide flu screening and treatment for the uninsured population served by Mercy Health Center in Athens/Clarke County and the 5 surrounding counties. Funds will be used to purchase 150 flu shots to protect patients from becoming ill. Funds will also be used purchase 100 flu tests that will be used to properly diagnose patients if they become ill. An additional $1000 is requested for the purchase of gift cards to be used to purchase Tamiflu. Tamiflu is used to treat symptoms caused by the flu virus and helps make the symptoms less severe and shortens the recovery time by 1-2 days.

DEO Clinic “Telemedicine and Coaching Calls to Overcome COVID Concerns While Monitoring Chronic Health Conditions” – $5,000 – Sponsored by Raymond Bedgood, MD

The phenomenal growth of the clinic is being stretched further in every direction by the loss of employment/insurance during COVID 19 (which is increasing patient numbers), and decreased availability of volunteers who must guard against infection. The clinic believes that innovation is the key to overcoming COVID and are thus seeking funds to utilize telemedicine and other creative solutions to keep patients on track, and to keep volunteers safely engaged with the work that they love during this pandemic.

The clinic has long-term volunteers who need to treat patients from home to eliminate the possibility of exposure to COVID. The patient population is low income, not all of them will have technology or access to the internet for participating in telemedicine. Patients who have the technological equipment can accept telemedicine calls at home. Patients who do not have smartphones or computers or internet can visit the DEO Clinic, have their vital signs checked by staff and use the clinic computers to have a telemedicine appointment with a provider who is volunteering from home. Patients who have smart phones but need wi-fi access will be directed to free wi-fi locations around the region if they wish to participate in telemedicine. DEO Clinic will provide patients with simple monitoring equipment to use at home. Blood pressure monitors, pulse oximeters, blood glucose meters, and scales with BMI measurements will be provided according to the patient’s most critical chronic conditions.

GHFA Awards Immediate Needs Grant Funding to Dr. Brandi Warren in Thomasville To Support Seniors in Need During Covid-19 Crisis

In response to the Coronavirus pandemic, the Georgia Healthy Family Alliance (GHFA), the philanthropic arm of the Georgia Academy of Family Physicians, awarded Dr. Brandi Warren of Thomasville, Ga. an Immediate Needs Grant to support fragile elderly patients in her area. Many of Dr. Warren’s patients are seniors and are not able to afford needed medications.  Fortunately, Dr. Warren can negotiate reduced rates with her local independent pharmacy for medications, such as insulin.  The $1,000 Immediate Needs Grant from GHFA will allow Dr. Warren to purchase critical, life-sustaining medication at or near cost and ensure that patients who have the greatest need will be able to access it.

Dr. Warren encourages all of us to be mindful of others during this ongoing crisis and to help where you can. “As a community we are uncertain of what lies ahead or how our daily lives will change in dealing with the COVID-19 pandemic,” says Dr. Warren. “Reaching out to our patients to say ‘I see you’ and ‘I care about you’ is so important, but to also be able to help by giving a vial of insulin or a case of Ensure can save a fragile patient’s life. Their blood sugar may normalize, their weight may stabilize, and they may step back from that dangerous ledge of depression they are teetering on from these small gifts,” she added.

GHFA’s Immediate Needs Grant Program Named 2020 Outstanding Program of the Year by the AAFP Foundation

The Georgia Healthy Family Alliance, the philanthropic arm of the Georgia Academy of Family Physicians (GAFP), has been named the Outstanding Program of the Year by the American Academy of Family Physician (AAFP) Foundation for the Immediate Needs Grants For Our Patients, program which was launched in late March in response to the COVID-19 outbreak in Georgia.

The Immediate Needs Grant program has provided more than $40,000 for micro-grants to GAFP member identified needs among their patients including prescription assistance for home bound senior citizens, lunch delivery for vulnerable children who no longer have access to school meals, PPE for charitable care clinics, and sanitizer distribution for Atlanta’s large homeless community.

“We are humbled to be recognized for our collective effort to help our patients and communities maintain health and hope during these uncertain times,” said GHFA Board President Dr. PJ Lynn. “I commend the GAFP members on the front line of this epidemic for going above and beyond to lift others in the face of this crisis,” he added.

AAFP Foundation President, Dr. Julie Anderson, congratulated GHFA for positively impacting Family Medicine with the Immediate Needs Program for our Patients project during the COVID-19 outbreak. The AAFP Foundation established the Outstanding Program Award to nationally recognize exceptional philanthropic activities of AAFP Chapters and Chapter Foundations. The Award is presented annually at the AAFP Foundation’s Annual Recognition Dinner during Family Medicine Experience (FMX) which was previously scheduled to be held October 15, 2020 in Chicago.

Current GAFP members including medical students, residents and active/ life members are eligible to apply for up to $5,000 to support GAFP member affiliated community organizations that support program priorities including underserved populations and community programs that promote healthy practices consistent with the principles of Family Medicine. The application deadline for the final cycle of 2020 is July 13th.  Grant awards will be announced in August 2020.

Your Help Needed – Send Out to Your Social Network – Help Raise Additional Funds for our Immediate Community Needs

Thank you for your continued support to your foundation, the Georgia Healthy Family Alliance. Your generosity continues to help make our mission possible.

As some of you may know, the GHFA Executive Committee approved an initial $25,000 (diverted from our Community Grants budget) to offer smaller grants (up to $1,000) for quick, immediate and urgent needs of member – identified community organizations in crisis due to the pandemic. We have distributed most of the initial $25,000 – and the Executive Committee has agreed to distribute an additional $15,000 – for a total of $40,000 to help our patients and neighbors with food, shelter and medical equipment.

If only $40,000 would cover the cost of requests that continue to come in from our members who are on the frontline and working with the homeless, hungry and frightened. But as we often say, “hope” is NOT a strategy – we have a PLAN – and with your assistance – we can quickly raise additional monies.  We have started a social media campaign using the Fundly platform and set a goal of matching the Alliance’s $40,000 to allow us to award more Immediate Needs Grants.

We are so grateful for physician members like past GAFP President, Dr. Susan Margletta Harmon, and her husband, for giving a donation in the amount of $2,000 to sponsor additional grants. Our ask of you is to give a $1,000 donation to also sponsor a much-needed emergency grant. Any amount you can give towards our COVID-19 relief fund is of course appreciated and will make an immediate impact on the lives of the people in our communities. No amount is too small.
